The Integrated Tools

Common Assessment Framework (CAF)

The CAF is a key part of delivering frontline services that are integrated and focused around the needs of children and young people. The CAF is a standardised approach to conducting an assessment of a child’s additional needs and deciding how those needs should be met. To access an eLearning package which gives you an introduction to CAF please contact Ann Lovegreen. E-mail: ann.lovegreen@darlington.gov.uk

Lead Practitioner (LP)

Lead Practitioners work with children and young people with additional needs that require an integrated package of support from more than one practitioner. The LP takes the lead to co-ordinate provision and act as a single point of contact for a child and their family. It ensures that professional involvement is rationalised, co-ordinated and communicated effectively and provides a better experience for children and their families involved with a range of agencies.

Information Sharing

Sharing information is vital for early intervention to ensure that children and young people get the services they require. It is essential for safeguarding and protecting the welfare of individuals and for providing effective and efficient services that are co-ordinated around the needs of individual children.
