What is an Assessment?
We carry out an assessment to find out what help you might need. An assessment means we work with you to identify your needs and to determine what services, if any, are suitable to support your needs. You will be provided with information about the services available and will be involved at all stages of the assessment. Carers can request a separate assessment of their own needs.
You have a legal right to an assessment of your needs if:
You feel you need help to live in your own home
You feel you may need to move into a residential or nursing care home
You are a carer for someone

What happens during the assessment?
When you contact Health or Social Services or someone contacts them on your behalf, some basic information about your circumstances will be needed to identify how best we can help you. We will arrange for an assessment to be carried out by the most appropriate person for you. This may be the district nurse, care manager, intermediate care assessor, hospital staff or continuing health care staff.

What happens after the assessment?
If you are assessed as needing services and have agreed with us what help you need, we will provide you with a copy of your care plan. This is the document which provides information about the services you will receive, who will provide them, where, and when. If you are provided with any services, we will remain involved in making sure that the services you receive continue to meet your needs.
Remember - You can ask for the services you receive to be reviewed at any time if you feel they are no longer meeting your needs.
