Intermediate Care

Intermediate Care is a range of services designed to help people recover from illness, surgery and medical treatment, and prevent avoidable admission to hospital and long term residential care.


These services are for people aged 50 years or over who live within the borough of Darlington and may include:

  • You returning home from hospital with a "package of support". This is where your support needs are provided at home by professionals from Health and Social Care.
  • You visiting or staying at a rehabilitation unit for a short period of time and professionals from Health or Social Care will provide services to support you to be as independent as possible.

It can also be a combination of the above.


Support from Intermediate Care Services is available from as little as 1-2 days up to a maximum of six weeks depending on your needs. This service is free up to a maximum of six weeks, as it is part of your health care treatment.


District Nurses, your GP, a care manager from Adult Social Care or the Discharge Management Team from the hospital can arrange for an assessment if it is identified that you would benefit from this support. The Intermediate Care team will be contacted and a care professional will visit you to complete a detailed assessment and agree a support plan.
