When a person with a physical, learning, sight or hearing impairment reaches the age of 18, the responsibility for any social care support they need transfers from Children's Services to Adult Social Care.

Your parents or carers will also have the opportunity to have their needs for support assessed. They can have their needs assessed jointly with you or they can ask for a separate carers assessment.

When a young person reaches adulthood the support needed may change. This could mean that they are no longer eligible for the same support, or that more support is required for them or their carer.
You may also need support from someone to help you speak up for what you need.
There are a number of options available for you to choose how the support you need is provided.
