We have a team in Darlington of Social Workers and Personal Advisors based at Central House who provide a service to looked after children age over 16. They help with making pathway plans to plan for when young people are ready to leave care and how they will be cared for before they leave, they support foster carers and work with schools ,colleges and employers to make sure care leavers gain maximum chances to do well once they have left care. They also help care leavers when they are living in their own accommodation and learning to manage budgets, live independently and take responsibility for being healthy and safe.
The team work with young people until they are at least 19years and often older depending on the needs of the young person and their wishes
