A referral form can be completed by a social care or voluntary organisation in Darlington.

An assessment of needs will be carried out to find out how you would benefit from Housing Support.

Details of social and medical background, the reason for the referral and the support/care needed by you must be provided.

If accepted, you will be put on the waiting list and offered a place as quickly as possible.
