Free Advice from your GP Surgery

Employed by Darlington Citizens Advice Bureau, an advice worker offering advice on a wide range of subjects will call at the Doctor’s Surgeries listed below.

If you have any issues you would like to discuss with the CAB worker an appointment can be made for you through your Surgery.

Please note that you may only use the GP outreach service if you are registered at the Surgery where the service is being provided.

Provides a programme of healthcare to older people in their own home delivered by Health Care Assistants, Therapists and District Nurses.

Immediate Response Team - Tel: 01325 743488

Provides rapid access to packages of home care for older people during a time of crisis for a period of up to seven days.  The service is accessible 24 hours a day/seven days a week

Supported Discharge Service - Tel: 01325 743488

Provides packages of home care for up to 4 weeks after a hospital stay, which aims to enable the older person back to independence.

Hundens Rehabilitation Unit - Tel: 01325 743488

Provides healthcare and rehabilitation which aims to restore the older persons back to independance..

Darlington Respite and Recovery Scheme - Tel: 01325 743488

Provides six beds in Eastbourne Nursing Home for older people with health needs who require no longer than a 2 week episode of 24 hour care.

District Nursing Services

Offers home based care to all people with health needs that require professional nursing care and are not able to visit their GP's surgery, or do not require hospitalisation.   The service is available 24 hours a day.

Traditionally works with children under 5 years of age, however, health visitors also have an important role in promoting health in older people.

Provides domiciliary advice, support, information for all adults and can be accessed via a care professional referral.

Is available to all adults with a physical neurological disability and provides community rehabilitation.

Is available to all adults with foot health needs.

District Nurses Offer assessment, advice and treatment in the promotion of continence.
