Leaving Hospital

For those patients who will need help from Adult Social Care and health care professionals when they leave hospital, support is available in the community.

 While you are in hospital the Adult Social Care Discharge Management Team and health care professionals will work with you and your carers/family to assess your needs for support in readiness for when you are medically safe to leave hospital. There are a variety of services that can be organised in discussion with you depending on your needs and circumstances.

  • For people returning to their own home but who will need support when they return, the options available, such as Reablement can be found on the Support to live at home webpages
