Level 1 & 2 Bikeability training is offered at your child's school, you should have already received and read the information leaflet from school giving you details on the training

Your child must bring a road-worthy cycle and helmet. A high visibility waistcoat will be provided by us and needs to be used for each lesson. NO training can take place without a cycle and helmet.
A reminder text message will be sent prior to your child's Bikeablity lesson, please supply a mobile number in the contact section below. Please note: contact details are not shared with third parties and will be destroyed at the end of the training programme.
To facilitate cycle training bikes and helmets available if required. Any loaned cycles and helmets will need to be stored securely for the duration of the training

Please give us any information you think necessary to assist the training and safety of your son/daughter. Include allergy or other current medication as appropriate.
In the event of an emergency, if I am not contactable on the phone number provided I consent to the child named on this form to receive medication or treatment deemed necessary from the appropriate authorities present.
We may take photographs/videos during training events and use them for instructor training purposes or on the Council / Let’s Go Tees Valley / Connect Tees Valley website(s), our social media pages, in the local press and in promotional materials.
